How does Record Finder work?
Log into ChartSwap to start a Record Finder search
Navigate to the Record Finder tab where you have the ability to submit a search containing patient demographics including name, DOB, gender, and ZIP code. Enhance your search by also including the last 4 digits of the SSN, phone, and/or city/state. Once the search is submitted, ChartSwap will search for patient records.
Patient demographics are searched to identify matches
ChartSwap will perform a search of thousands of healthcare locations across the country and will notify you once results are available via email within 24 hours.
View search results
Log into ChartSwap and navigate to the Record Finder page. After the ChartSwap provided patient authorization is submitted, the provider details will be visible for your review. The search results will include a listing of locations where the patient received care and the record types available for retrieval.

Select the records you would like to retrieve – you can select a single record or as many as needed.
