Using Record Finder is simple:
- Log in to ChartSwap to start your record search with patient demographic information.
- ChartSwap uses the patient demographics to search thousands of healthcare locations around the country. You’ll receive a notification with your results within 24 hours.
- Provider details and a list of locations where the patient received care will be available for viewing after you submit the ChartSwap-provided patient authorization.
- Select the records you’d like to retrieve.
- ChartSwap retrieves your records and notifies you once they are available in the portal.
The ChartSwap provided authorization is designed to be applicable to all potential providers, you do not need to list a specific provider. This allows ChartSwap to search for any provider from whom your subject may have received treatment. Because Record Finder searches for matches spanning five years, the ChartSwap provided authorization includes a statement that covers this entire period. If any other date or period range is entered, the authorization will be rejected.
