Discover the unknown with Record Finder

Our goal has always been to provide our clients with a fast, simple, and secure solution for streamlining their record requests. As part of that mission, we pride ourselves on listening to customer feedback and suggestions to improve the ChartSwap experience whenever possible. So, when our clients began asking for a way to find additional records, we set to work on developing our new Record Finder capability.  

On average, there are 1 billion physician office visits and over 139 million emergency department visits in the U.S. each year. Our own data suggests that the average patient has records in seven to eleven different provider locations. With so many possible records, it can be hard to know all the locations where your subject may have received treatment. Record Finder uses your patient’s demographics such as name, date of birth or zip code to search our nationwide network of providers for a comprehensive list of locations that may have records. This enhanced search capability makes it easier than ever to track down previously unknown records that could contain vital information for your case. It’s all made possible by the ChartSwap provider network, which is the most extensive network of its kind in the U.S. Our provider network includes over 1 million providers and over 661K clinical facilities. 

Using Record Finder is simple:  

1. Log in to ChartSwap to start your record search with patient demographic information. 
2. ChartSwap uses the patient demographics to search thousands of healthcare locations around the country. You’ll receive a notification with your results within 24 hours. 
3. Provider details and a list of locations where the patient received care will be available for viewing after you submit the ChartSwap-provided patient authorization. 
4. Select the records you’d like to retrieve. 
5. ChartSwap retrieves your records and notifies you once they are available in the portal.   

The ChartSwap provided authorization is designed to be applicable to all potential providers, you do not need to list a specific provider. This allows ChartSwap to search for any provider from whom your subject may have received treatment. Because Record Finder searches for matches spanning five years, the ChartSwap provided authorization includes a statement that covers this entire period. If any other date or period range is entered, the authorization will be rejected.
