We are now fulfilling all third-party medical and billing record requests electronically through ChartSwap.

ChartSwap offers a fast, simple, and secure platform that facilitates the electronic exchange of medical and billing records between healthcare providers, law firms, and other authorized requesters.

Registration is free, and requestors can easily use ChartSwap to submit, track, pay for, and download records—all in one convenient place.

Requests via mail or fax or email are no longer processed, please re-submit this request via ChartSwap.com using the following steps.

  1. Register at http://www.chartswap.com/register as a Record Requester
  2. Sign in and Search for a Provider, Enter Request Details, then Upload Supporting Documents
  3. Once your request has been reviewed and records are available, you will receive a notification and invoice which you can pay with a check or credit or debit card.

How does ChartSwap use patient records?

It’s simple: We don’t. ChartSwap only provides a secure online platform for requestors and providers to exchange patient records. ChartSwap will only access patient records for a limited number of reasons, such as when it is required by law or in the process of fulfilling a customer service request.

Is there a cost for a requester to join ChartSwap?

No, ChartSwap is free to join and requesters pay for records they request from the custodian based on their state and local policies and guidlines.
